General Rate
and Service Terms
1. Calls that begin in one rate period and terminate in another will be
billed for the entire call duration at the rate applicable at the
commencement of the call.
2. The duration of each call for billing purposes will be rounded off to the
nearest higher billing increment. All services are billed in whole minute
increments.
3. Any rate application that results in a charge expressed in fractional
cents will be rounded up to the nearest whole cent.
4. If a fixed monthly service charge applies to a plan that has both
intrastate, interstate, and international traffic eligibility, the fixed
charge will be deemed to be interstate for purposes of assessing a single
fixed charge.
5. Monthly fees are billed in advance. Service fees will be prorated for
service periods that are less than a full month's billing cycle. For
purposes of determining prorated amount, each month is assumed to have
thirty (30) days.
6. Unless otherwise indicated, all usage variable charges will be billed in
the following billing cycle.
7. State, federal, sales and all other applicable taxes apply to all calls.
8. The Company does not charge other surcharges and fixed fees beyond those
indicated in the rate schedules shown.
9. Rates and terms subject to change.
10. All services are subject to availability.
IntraLATA Optional
Calling Plan
Optional Calling Plan 1 and 2
Plan 1 and 2 are optional calling plans that allow subscribers to either
plan to recieve a flat rate per minute ("Plan Rate") to the exchanges
indicated with each plan and an extended flat rate ("Extended Rate") for
calls outside the Plan calling area. Each Plan requires payment of a flat
monthly fee ("Plan Fee").
1. Plan exchanges are subject to change.
2. These plans replace other plans contained elsewhere in this tariff and
cannot be combined with other plans.
3. All calls are billed in whole minute increments and rounded up to the
next highest one-cent increment.
4. Rates do not include taxes and surcharges.
5. Participation in a plan requires presubscription to the company and
cannot be provided on a dial around (1010XXX) basis.
6. Payment of telephone company fees (PIC change, etc.) is the
responsibility of the subscriber unless part of a Company-promotion.
7. Plan Rate only applies to exchanges listed in 4.9C(8)(d) and
4.9.1C(9)(d). All other exchanges not listed but within the customer's LATA
will be subject to the Extended Rate.

Direct Dial and Calling Card
Direct Dial Calling
1. Direct dial calling services are available on interstate, interLATA and
intraLATA calls or on interstate and interLATA calls only. Service is not
available solely on intraLATA / home region toll calling.
2. Customers will be advised if their selection of toll and interLATA toll
providers create a conflict with the Company's routing system.
3. Local exchange carrier primary interexchange carrier charges may apply
for change in underlying carrier. The company may, as part of a generally
available promotion, waive or reduce such charges on behalf of its new
customers.
4. Optional Calling Plans are available to those customers who meet term,
volume or other stated eligibility requirements.
Rates for International calls are found under "international calling"
Calling Card
1. Calling cards are the property of the Company and must be destroyed or
returned to the company upon request.
2. Customers are responsible to the extent limited by law for any charges
billed to a lost or stolen card.
3. The Company reserves the right to disable cards suspected of fraudulent
charge activity.
4. Cards are issued with international calling capability disabled. This
ability can be activated upon request however customers so requesting such
activation do so with the understanding that they remain responsible for
higher rates charged to these locations.
5. The ability of callers to complete calls is contingent upon proper
routing by the originating end user location and serving telephone company.
The Company cannot guarantee completion of such calls from all locations.
6. The payphone surcharge will apply on calls originated from lines with a
"payphone designation" that deems them eligible for the assessment of dial
around compensation.
7. Operator surcharges apply to calls completed with the assistance of a
live or recorded operator service.

Optional Calling Plans
Optional Calling Plans are available to customers who meet stated
eligibility requirements and are presubscribed to the Company for service.
When services are restricted to residential or business classification
restrictions, determination of residential or business classification will
be pursuant to the classification of the service line by the underlying
local telephone service provider. The Company reserves the right to refuse
service to any subscriber for any reason including failure by the customer
to meet eligibility requirements.
Subscribers may choose InterLATA only or interLATA and intraLATA options.
Services are not available on an intraLATA basis.

customers of the plan pay the same rate for services indicated regardless of

Toll Free Services
Toll Free Services
1. Toll-free numbers may be assigned randomly or from a list of available
numbers provided by the Company. The Company makes no warranties about the
availability or use of a number by prior or future owners of a toll-free
number.
2. The Company will attempt to honor requests for specific number(s) but
assignment and availability of such numbers is not guaranteed. Company
reserves the right to limit the number of number-assignment requests.
3. Release of a Customer toll-free number to another provider is contingent
on payment of any current outstanding charges owed to the Company and
accurate completion by the receiving carrier of Responsible Organization
transfer forms.
4. In the event of a dispute between Customers regarding the ownership of a
toll-free number, the Company reserves the right to determine ownership
based on previously provided application material, however, in the event
that the Company determines that ownership is unclear it will in no way be
held liable for such ownership determination.
5. The Customer is responsible for payment of misdialed calls to the
Customer's toll-free number.
6. Non-recurring charges apply per order placed.
7. The ability of callers to complete calls is contingent upon proper
routing by the originating end user location and serving telephone company.
The Company cannot guarantee completion of such calls from all locations.
8. The Company reserves the right to waive non-recurring charges for those
customers who commit to a one year term or have usage billing within
three-months of service totaling $25 per month or more.
9. Toll-free numbers cannot terminate in international locations, other
toll-free numbers, or numbers prefixed with 700, 900, or 500 NPA. The
Company reserves the right to refuse termination of toll-free services
outside its designated operating territory.
10. Toll-free PIN access numbers are the sole property of the Company for
use with its PIN service.

1. The Company does not knowingly bill for unanswered calls however in some
instances, equipment in foreign countries may give false or unreadable call
answer supervision. Calls in excess of 60-seconds in length will be presumed
answered.
2. Rates shown include additional charges for special service terminations.
These termination rates are revised frequently; the company reserves the
right assess special services surcharges as a separately identified line
item on the bill should the assessment of those charges warrant it doing so.

Directory Assistance (DA)

provider. The Company does not warrant the accuracy of listing information.
2. DA services are billed per access not request. Calls that are cut-off or
fail to provide accurate information will be billed. The Company reserves
the right upon request to credit such charges for inaccurate or missing
calling data.
Operator Services
